Secure Online Patient Portal Now Available!

Our patient portal is a secure online website that gives our patients 24-hour access to personal health information. The portal is located at: https://www.followmyhealth.com

To obtain access to our patient portal system, please complete the following steps:

1.  At your next visit to our office, provide your e-mail address to the front desk receptionist.
2. Within 24 hours, you will receive an email with your invitation to our patient portal.
3. Click the registration link in the email you receive from noreplay@followmyhealth.com
4. Follow the steps to create your unique username and password.
5. Enter the invitation code provided by your Pinnacle Medical Group and click “Agree” to the release of information.

After you have completed registration, you will receive two additional emails:

1. One welcoming you to the portal
2. Another confirming your connections with our organization You will have 24/7, secure online access to our patient portal and your medical information!
