Rialto Office Moved Across the Street to 1851 Riverside Ave.

Pinnacle Medical Group’s Rialto office moved across the street to a bigger office, with more doctors, on August 23, 2014. The new address for primary and urgent care is:

Dr. Amal Ibrahim – Pediatrics and Dr. Jesse Guadiz – Family Medicine moved to the new location that came to Pinnacle Medical Group as a part of the merger with United Family Care. They are joining Dr. Jocelyn McGrath – Pediatrics, Dr. Keth Fu – Internal Medicine, Dr. Ayad Makar – Urgent Care and Dr. Ken Nguyen – Urgent Care all previously with United.

The Urgent Care Hours for Rialto remain the same:

Weekdays: 11:00 am – 10:00 pm
Saturdays: 9:00 am – 5:00 pm
Sundays/Holidays: 9:00 am – 4:00 pm

The new Extended Care Center at 8110 Mango Ave in Fontana that also accommodates walk-in patients, is open the same hours.
